Scientists have identified important differences in the small-bowel microbiome of women and men, raising the possibility that these variations could help explain why certain digestive and hormonal conditions are more common in women.
The study, conducted by researchers at Cedars-Sinai Health Sciences University and published in iScience, examined how microbial communities in the small intestine vary between the two groups. The findings could eventually support more personalized approaches to understanding and treating disease.

Small-Bowel Microbes Differ Between Women And Men
The small-bowel microbiome is made up of microorganisms that live in the small intestine and contribute to several essential processes, including immune regulation, metabolism and hormone-related functions.
Researchers found that women and men differed not only in the types of bacteria present but also in the way these microbial communities were organized.
One particularly notable finding involved a bacterial community containing Granulicatella. According to the study, this community was detected only in women.
Ruchi Mathur, MD, director of Clinical Research and Clinical Operations for the Medically Associated Science and Technology Program at Cedars-Sinai and corresponding author of the study, explained that this finding was especially interesting because bacteria from the same community appear at higher levels in women with polyendocrine metabolic ovarian syndrome, previously known.
Possible Links To Women’s Health
The discovery may be relevant because some conditions affecting metabolism, hormones and digestion occur more frequently in women.
The researchers did not establish that the identified bacteria directly cause these conditions. Instead, the findings highlight associations that may help scientists better understand why the small-bowel microbiome differs between women and men and how those differences might influence health.
The study also found variations in how microbes in the small intestine produce fructans.
Fructans are complex carbohydrates that can trigger digestive symptoms in some people, particularly those with irritable bowel syndrome, or IBS. This condition is considerably more common in women than in men.
Differences in how gut microbes process fructans could therefore offer another clue to why digestive symptoms do not affect everyone in the same way.

Why These Differences Matter
Mathur emphasized that differences between women and men should be taken into account when researchers study the small-bowel microbiome. She explained that considering these variations may become increasingly important as medicine moves toward more individualized ways of understanding disease and selecting treatments.
This approach could be especially valuable because the small intestine plays a major role in nutrient absorption, metabolism and interactions between the digestive and immune systems.
A Step Toward Personalized Medicine
The findings do not yet provide a new treatment for IBS, hormonal disorders or other conditions. However, they suggest that differences in the small-bowel microbiome deserve greater attention when researchers investigate diseases that disproportionately affect women.
Future studies will be needed to determine exactly how these microbial communities influence symptoms and whether they could become useful targets for diagnosis or treatment.
For now, the study adds to growing evidence that the microbiome varies significantly between individuals. Understanding how these differences relate to women’s health may help researchers build a more precise picture of digestive and metabolic conditions and eventually develop more personalized approaches to care.
